All volunteers who drive their own personal vehicle for sponsored activities of the Genesis Church must carry automobile liability insurance on their own personal vehicle. The following form will be required on an annual basis for all volunteer drivers.

By checking this box, I understand that my personal automobile insurance is the primary coverage.

Genesis Church’s automobile policy provides Hired and Non-Owned Automobile Liability insurance. This insurance is Excess insurance. All volunteers who drive their own personal vehicle for sponsored activities of the Genesis Church must carry automobile liability insurance on their own personal vehicle. The volunteer’s personal automobile insurance will respond first in the event of an automobile loss. Genesis Church’s insurance will respond for an insured loss, but ONLY AS EXCESS LIABILITY INSURANCE. I have read this information and automobile insurance is provided on the personal vehicle driven for this sponsored activity of the Genesis Church. I also understand that the insurance coverage of the Genesis Church is Excess insurance and is not Primary insurance.
